The Broome Regional Aboriginal Medical Service (BRAMS) is an Aboriginal Community Controlled Health Service. Commencing in 1978, BRAMS was the first remote Aboriginal Medical Service in Western Australia. BRAMS is governed by a Board of Directors elected by its members.

BRAMS provides comprehensive, holistic and culturally responsive primary health care services to Aboriginal people living in Broome.

About the Opportunity

BRAMS have an exciting opportunity for an experienced Disability Services Case Coordinator to join their close-knit team on a full-time basis.

BRAMS is seeking a person who is reliable, having a caring and compassionate personality, are flexible and willing to work with a wide range of people. The Disability Services Case Coordinator provides assistance to NDIS participants to understand what is included in the NDIS plan, and supports participants to identify goals and needed supports and to choose and connect to suitable Disability Support Providers. The Case Coordinator can also assist the participant to strengthen their abilities to advocate for and coordinate supports and participate in the community.

More specifically, your responsibilities will include (but not be limited to):

  • Understands person centred approach and enables participants to implement plans in a customised and practical way.
  • Provides support for participants to understand their plan, identify goals, supports and appropriate providers.
  • Uses excellent interpersonal and communication skills to assist participants to understand plans, service providers and supports available.
  • Ability to work individually and effectively manage a case load of participants, also work collaboratively to ensure appropriate supports are available.
  • Innovative approach to supports to enable participants to blend informal, mainstream and paid supports to achieve tailored supports.
